The patient priority client problems are: Fractured ribs and Atelectasis as a result of his recent fall. Clinical manifestations: The patient is using an 8L oxygen mask and has emphysematous alterations throughout his lungs, which indicates breathing difficulties. The patient stated having severe pain at the location of the fractured ribs making it difficult to breathe deeply, resulting in atelectasis. Nursing diagnosis : Ineffective breathing Pattern related to fractured ribs as evidenced by the patient`s report of pain when breathing, and the use of 8L oxymask indicating difficulty in breathing. Nursing interventions: Positioning: Position the patient in a way that maximizes lung expansion, such as semi-Fowler’s or high Fowler’s position. Breathing Exercises: Teaching and encouraging the patient to perform deep breathing exercises and use incentive spirometry. This can helps to improve lung function. Regular Assessment: Regularly assess the patient’s pain level, respiratory status, and overall comfort. This helps to detect any changes in the patient’s condition early for immediate attention.
