The patient is an 80-year-old Caucasian male who is brought to the clinic by his wife. The wife states that her husband is deteriorating in regards to his cognitive status. She states their grandchildren visited recently, and the patient was confused and could not recall their names. Additionally, she informs the nurse practitioner that her husband has been experiencing significant difficulty in activities of daily functioning (eating, bathing, and dressing herself independently). She begins to cry and tells the nurse practitioner, “What am I going to do? There are days that he doesn’t even know me.”
