A patient has ambulated to labor and delivery reporting bright red spotting for the last hour. On the monitor, the nurse notes a baseline FHR of 165, minimal variability, and recurrent late decelerations. She is a G3P2 at 39 weeks and 3 days. What are the nurse’s next steps? Turn the patient on her left side, call the provider, place her on O2, and start an IV to give a fluid bolus. Discharge the patient from OB triage; these are normal findings. Take a complete medical history and then inquire as to how long she’s been taking illegal drugs. Inform the patient that she is having an abruption and prepare her for surgery.
