All the symptoms and feelings C.H experiencing is because of being dehydrated. The dizziness, headaches, confusion, restless, low blood pressure, fever/chills occur due to her dehydration. Having not enough fluids in the body, it is hard to maintain a regular body temperature which leads to dizziness, fever, confusion, headache, blood pressure drop and restlessness. So the nurse should address her water intake issue. And yes, monitor CH’s intake and output especially the sodium intake and recommend food rich in water. The nurse can also suggest to the physician for follow up labs to assess improvement in CH’s hypernatremia.
