Real World Case 3.1: Riverside Medical is a multi-physician practice that specializes in chronic disease management. The providers are interested in collecting patient-generated health data (PGHD) via the patient portal to assist in treating diabetic and hypertension patients. They are unsure how to obtain these data and incorporate them into their daily workflow.
