ER Admission Note CC: Altered mental status HPI: Police were called to a public park where a young man was shirtless and acting bizarrely. The man, who appeared confused, was pacing and gesturing as if he was hallucinating. Paramedics were called because of his behavior and the patient was brought in to the local ED.
