59-year-old with abdominal pain described as upper abdominal specifically in the right quadrant. She has experienced nausea and vomiting. Also states she has not been eating as her appetite is decreased significantly. No history of heartburns, ulcers, but does take TUMS on occasion. Had a C-Section approximately 5 weeks ago. Past medical history significant for Factor V coagulopathy. Medications: Coumadin, every other day 5 mg, 7.5 mg alternating days. CBC were normal as well as abdominal series ASSESSMENT: RUQ pain PLAN: Follow closely, could be GB or pancreatitis. Will order GB ultrasound on an outpatient basis and she will return immediately if symptoms worsen. icd-10-cm
