Reword. SW contacted pcg to reinterate that all medication /medical questions are handled by skilled nursing. SW reminded patient that SN handles mailorder medication. SW explaiined to pcg to be sure to follow nurse recommendation of picking up medication in case any inclement weather. pcg stated she misunderstood and it doesn;t take much. sw explaied to pcg that is complely understandable because she has a ful l Caris team providing care and it is possible to get information mixed up. Pcg inquired about life alert and stated pt stated she might need life alert button in case she falls. Sw informed pcg that she will help her setup life alert services next week. PCg stated that would be ok.
