Patient Name: Carla Smith Age 65 Chief Complaint: loss of appetite, bloating, nausea, vomiting, abdominal cramps, diarrhea. Hospital Admitting Diagnosis: gastroenteritis Past Medical History: Type 2 Diabetes Mellitus, asthma, hypertension Shift Report: Patient was admitted to the hospital 3 days ago with nausea, vomiting and diarrhea for 3 days. She was diagnosed with gastroenteritis and treated IV fluids and anti-emetic medication. Her symptoms have improved and the diet was advanced yesterday to a regular diabetic diet. She tolerated lunch and dinner yesterday and it is anticipated that she will be discharged later today. Her medical history includes Type 2 Diabetes, asthma, and hypertension.
