A 36-year-old presents to the emergency department with right arm weakness, right side facial numbness, and difficulty speaking, which developed an hour before presentation. The patient also has had a fever, cough, and headache. The patient is admitted for possible CVA which is ruled out after study. Workup did reveal thrombocytopenia with a platelet count of 11,000. The diagnosis of thrombotic thrombocytopenic purpura (TTP) is made based on the thrombocytopenia, fluctuant neurological findings, and fever and the patient undergoes plasma exchange.
