This 93-year-old patient with a history of hypertension reports a history of fever and chills for approximately the past 7-10 days. He also reported a headache and lack of appetite. He presented to the ED at which time a chest x-ray showed extensive pneumonia. Blood pressure was normal as were other vital signs. No adenopathy. Lungs had rales and some wheezing. Heart regular, no murmurs. Abdomen is soft and nontender. Extremities appear normal.
