Reword. Sw also informed Patient that the program choices that she was referred to to get help in thehome has called three times trying to reach her. Sw explained to patient the importance of answering the phone to ensure in home supportive services can be placed in the home. Patient reported that sh ehas been paying almost $200 for her medications. Sw exlained that hospice meds are covered by Caris. Sw spoke with Sn and inquired why patint has been paying for her medication. Patient Care Manger reported pt is not giving optum crd to the pharmacy. SW explained to PCM that patient is an amputee and the Caris team should be ensuring the pharmacy has the prescription card. SW had to leave for meeting but will follow up with pharmacy and send optum pharmarcy card.
