Today you are working at Northside Community Centre as a community support worker. You are having a follow up meeting with your client Valerie to discuss her Care Plan. This time she is accompanied by her daughter Amy who is also her carer. Valerie has recently been diagnosed with Diabetes and you are in the process of organising for Valerie to attend an information session at Ashton Diabetes centre. You advise Valerie and Amy that the next session is Wednesday at 3.30pm. Amy lets you know she would like to attend the Diabetes information session with her mum however she cannot attend a late session as she has to pick up her children from school.
