M.S. is a 58-year-old patient, presented with a chief complaint of burning chest discomfort. The patient described the pain as a 4-5/10 in severity, which radiates up into his neck. The discomfort lasts for approximately 30 minutes. He also reported having one alcoholic drink post-dinner, but not every night. The patient was asked about other symptoms such as fatigue, difficulty sleeping, unintentional weight loss or gain, shortness of breath, wheezing, difficulty catching breath, nausea, vomiting, constipation, diarrhea, and coffee grounds in vomit. He was also asked about his diet and any history of heartburn. The diagnosis was Gastroesophageal Reflux Disease (GERD), with differential diagnoses including Unstable Angina, GERD, Pneumonia, and Aortic dissection.
