A 63-year-old woman presents to you with a history of intermittent chest pain over the past few months. The pain appears with moderate exertion and she describes it as a pressure sensation on the left side of her chest accompanied by some numbness of her left hand. She smokes cigarettes (25 pack-year history) and is also complaining of chronic cough and expectoration of nonpurulent sputum. She occasionally uses an albuterol inhaler but never had pulmonary function tests performed. Of interest, she also has chronic diarrhea and at one point was diagnosed with Crohn disease, but she does not take any medications for this condition. She also has intermittent heartburn and uses omeprazole and calcium carbonate as needed, which she tells you relieve the symptoms.
