Kate Wilson is a 3-year 10-month-old girl who was seen by a pediatric nurse practitioner (PNP) in the office two days ago for right ear pain. Evaluation by the PNP revealed slight redness of the right tympanic membrane and scant fluid behind the membrane. A sharp light reflex was present. Her temperature was 99.7o F (37.6o C). It was determined that, because of her age and current symptoms, further observation and oral analgesia were recommended. The mother was educated on the signs and symptoms to watch for that would warrant a return to the office. Today Kate returns to the office complaining of worsening ear pain. No analgesics/antipyretics have been provided. Her mother states earlier on the morning of the return visit, Kate had a measured fever of 102.4o F.
