You enter the room and complete your initial physical assessment. Neuro: WNL. CV: WNL+ 2 pulses in all extremities. Capillary refill < 3 seconds. Resp: Breath sounds diminished with saturations 92% on room air. GI: bowel sounds hypoactive all quadrants. GU: Has not voided since returning from OR 8 hours ago. Integ.: Pink and intact except for IV 20 gauge in the right forearm infusing 0.9 NaCL at 150 mL/hour. IV site w/o redness or edema; puncture wounds from surgery are intact with steri-strips. MS: WNL. Pain: 5/10 in her abdomen. VS: BP 144/70, Temp 100.6, Heart rate 96, Resp rate 10, O2 sat 92% What concerns with your post-operative assessment?
