Treating new AF patients with the appropriate OAC in a timely manner is plausible in the acute care setting. One way this could be done is by completing a CHA2DS2-VASc score on admission for new AF patients. This could be done through a daily reminder in the EMR or in a care plan bundle for each AF patient. This will not only provide education to the patient about their new diagnosis and comorbidities, but help the medical staff develop on an ongoing hospitalization plan of care for the patient. Nurses can also initiate early education and discharge planning for new AF patients with this information.
