Topic: Fluid and Electrolyte Imbalances

This Discussion topic is based on a case study. As in all case studies, review the facts of the case and consider the various steps of the nursing process in order to address the critical thinking questions.

Case Study:

W.T. is a 74-year-old Caucasian female with a history of hypertension and type 2 diabetes mellitus. She is being assessed at home by the home health nurse because she is having difficulty urinating and states that her urine is dark, “like tea.” Her vital signs show tachycardia at 110 bpm, her BP is 100/54, RR is 20 and shallow, O2 sat 95%. She has lost four pounds since her last home health visit three days ago. The home health nurse determines the client should be taken by ambulance to the hospital.

What laboratory tests do you anticipate the physician ordering for W.T. upon admission to the hospital?
As the nurse, what would you anticipate W.T.’s laboratory values (i.e., basic metabolic panel) to show?
What vital signs do you expect to find for W.T.? What causes this?
What type of fluid and/or electrolyte imbalance(s) do you anticipate based on W.T.’s signs and symptoms?
What would be the appropriate type of IV fluids to start this client on and why?
Explain the signs and symptoms you would expect to assess in regards to W.T.
Using the nursing process, describe the nursing interventions you would anticipate providing for the client.
What is the role of the home health nurse upon arrival to the emergency department with the client?
