Case Study:
W.T. is a 74-year-old Caucasian female with a history of hypertension and type 2 diabetes mellitus. She is being assessed at home by the home health nurse because she is having difficulty urinating and states that her urine is dark, “like tea.” Her vital signs show tachycardia at 110 bpm, her BP is 100/54, RR is 20 and shallow, O2 sat 95%. She has lost four pounds since her last home health visit three days ago. The home health nurse determines the client should be taken by ambulance to the hospital.
