Case 2 CC: MK is a 41 yoF presenting for evaluation of recurrent diarrhea. HPI: MK states that 1-2 days per week she will have around 5 BMs with a loose consistency/appearance. MK reports that this has been ongoing for the past 6 months. She states that her on her “good days” she has a normal (not loose) BM once every morning, and this was her baseline prior to the past 6 months for as long as she can remember. MK denies weight loss, dark/tarry stools, FH of colon cancer, personal history of IBD or iron-deficiency anemia. Labs: All of MK’s labs are wnl. PMH: Epilepsy, tobacco abuse, anxiety, obesity (BMI 41) ALL: penicillin, aspirin, watermelon Medications: • Levetiracetam 500mg PO BID • Diazepam 5mg PO QHS prn anxiety 3. One year later, MK presents to clinic for an annual check-up. She reports taking alosetron for her IBS-D successfully for 6 months until her insurance stopped covering the expensive product. Fortunately, she has been able to prevent IBS-D symptoms without any medications by avoiding spicy foods and limiting dairy products.
