Ms. C is a 30 year old female who is employed as a telephone installer for a major cable company. She is 5’7″ with current weight of 200 pounds. She is presently pursuing a master’s degree in telecommunication. Ms. C is being seen at her family physician’s office for a follow up review of observed patterns of behavior changes and changes in her health status over the past month. During triage, Ms. C reports unusual hunger, even after having a large meal. She has noted unusual weight loss that is not related to exercise or stress. She also reports diarrhea and the inability to tolerate normal heat that she would have responded to in the past. On assessment, her vitals are: BP 150/92, P129, Rep22, T 101.6 F. She has visible tremors of the hand and her eyeballs are beginning to protrude. Current diagnostic studies reveal abnormal T3, T4 and TSH results. A thyroid scan with the use of radioactive tracers reveals an enlarged thyroid goiter with increased iodine uptake. Ms. C denies any difficulty swallowing or pain upon swallowing. A diagnosis of hyperthyroidism is made by the physician and is discussed with Ms. C and the possibility for surgery
