16-16 Radiation Oncology Follow-up Note Print E/M Audit Form Print LOCATION: Outpatient, Clinic PATIENT: Morris Collette PHYSICIAN: James Eagle, M.D. HISTORY: Mr. Collette is a 69-year-old male who has been diagnosed with a Stage II (T2b, N0, MO) adenocarcinoma involving the right lobe of his prostate associated with a Gleason score of 8 and a pretreatment PSA of 6.5 ng/ML. Mr. Collette has been treated with androgen blockade with IMRT radiotherapy to a total dose of 78 Gray. He reports that his energy level, bowel and bladder habits have improved and are near baseline. He does have nocturia x 1-3 which is unchanged. He continues to have intermittent hot flashes associated with his Lupron injections. He was due for a Lupron injection in June and has inquired whether he needs this or not. He has no other questions or concerns at today’s appointment. ROS: The following toxicities were assessed as a 2: Fatigue – Pt reports “moderate” fatigue. Pain – Pt prescribed methodone 5 mg tablets take 2 tabs tid but he reports he doesn’t take them quite that often. Pain is rated as “3”110 to the neck. The following toxicities were assessed as a 1: Hot Flashes/ Flushes –
