Chief Complaint: “Headache and dizziness. My eyes are feeling off.” HPI M.B. is a 39-year-old female who is here with complaints of headache starting 4 days ago. Pt reports she was working on her computer and in her usual state of health when suddenly she became dizzy and vomited. Pt thought it was vertigo, so she notified her PCP and received a prescription for meclizine 25mg which she has taken for 2 days with no relief in symptoms. Today, pt states she is still having a left sided headache rated 6/10 in severity, does not radiate. Has been sleeping more but wakes up frequently because of the pain, sometimes rated 9/10. Eyes are sensitive to bright lights, denies vision loss but feels like her depth perception is off. Movement and positional changes make the pain worse. Reports a PMH of migraines and states this pain does not feel like her typical migraines because she can usually catch them early and will go away within 12 hours after taking Excedrin. Pt works as an event and marketing director, reports increased stress from an upcoming project but “it’s nothing significant”. Pt is very concerned because this is not her usual presentation of
