History of Present Illness: Harry Sucio is a 75-year-old male who presented to the ED several days prior for an ischemic stroke in which he did not qualify for tPA. After treatment, he is preparing to be discharged to a Skilled Nursing Facility (SNF) for rehab and further care. Medical History Harry Sucio has a medical history of HTN, DM2, CVA with left-sided weakness, dysphagia, & high cholesterol. His stroke resulted in him having a feeding tube with plans to replace it with a PEG tube in the next month. He has been having an increased frequency of intermittent urinary retention and has a consultation with a Urologist in 2 weeks. He has allergies to morphine and iodine. Medication History Harry takes aspirin, clopidogrel, Novolog, and lisinopril. Personal/Social History Harry is a married gentleman who lives with his wife in a two-story home. He has 2 children and several grandchildren that live in the same city and are supportive. He is retired from a previous job in construction.
