As a nurse, I have a duty to protect and advocate for my patients. A confused patient requires a different approach, one that may include having a caregiver in the room for safety. At times, simple communication can help clarify the matter. The first step is to communicate with the physician and state the facts in a nonconfrontational manner. If the physician does not correct what has been charted, I would seek additional help from the charge nurse. The ethical principle of veracity which promotes truthfulness, honesty, truthful disclosure of information, and integrity can be applied to this scenario (Green, 2022). Organizational culture determines if employees feel safe enough to speak up. Errors that are not addressed or corrected can lead to negative patient outcomes. The Agency for Healthcare Research and Quality (2022) identifies the following areas that facilitate patient safety culture: communication about error, openness, organizational learning, overall rating on patient safety, response to error, staffing, management support for patient safety, teamwork, work pressure, and pace. These are all important because they affect healthcare delivery outcomes.
