Patient is an 89-year-old female who presents with abdominal pain, nausea, vomiting and weakness. She also has had a fairly substantial weight loss over the last year or so. Symptoms started getting worse yesterday. She denies fever or chills. Bowels have moved, although not substantially; she says she really has not eaten enough to have a bowel movement at this time. She denied any blood in her emesis. No blood in her stool. The pain seems to be in the epigastric area, although it seems to radiate a little bit to the right side. Denies pain into the back at all. She also denies chest pain. She has become weak, enough thai she has fallen. Falling has been a problem for her in the past, but she has not had a fall for quite some time. PAST MEDICAL HISTORY: Hypertension, cholelithiasis, depression, osteoarthritis, and CHF. She also has cholelithiasis but no cholecystitis. She reports the recent onset of Type ll diabetes mellitus. She reports an allergy to penicillin and amoxicillin. FAMILY AND SOCIAL HISTORY: Noncontributory except she does smoke. REVIEW OF SYSTEMS: Denies dysuria or any difficulty urinating. She denies fever, chills or any swelling in her upper or lower
