The patient is a 58-year-old white Hispanic male who presents for a routine evaluation. He is 70″ tall, and 202 lbs. With a body mass index (BMI) of 29, the patient is overweight; he also has central adiposity with a waist circumference of 40″. His cardiac exam and peripheral pulses are both normal. The patient’s medical history is significant for hypertension (HTN) for which he takes Lisinopril/hydrochlorothiazide (HCTZ). His blood pressure (BP) on medication is 130/80 mm Hg. He has a significant family history in that his father had HTN and died of a myocardial infarction (MI) at age 72 years. His mother is 74 years old and was diagnosed with type 2 diabetes mellitus (DM) 16 years ago. She has some peripheral neuropathy. His sister is 50 years old and has HTN and a history of gestational diabetes. The patient is married with no children. He doesn’t smoke or consume much alcohol, and he gets irregular exercise.
