CC: Decreased urinary output and lower extremity edema HPI: Carolyn Sims is a 56 year old established patient, who presents to the clinic with decreased urinary output and lower extremity edema. Three months ago, she noticed voiding fewer times per day, and progressive lower extremity edema. Carolyn has also notices mild itching and looking pale. Occasionally, her urine looks like there is foam present. Carolyn has a 10 year history of diabetes mellitus. She does not routinely check her blood sugars and “tries to eat right”; although can be too expensive.
