A 64-year-old, 197 lbs, 73 inch male patient was admitted three days ago for a COPD exacerbation. He is currently receiving supplemental oxygen via nasal cannula at 3 L/min; albuterol and ipratropium every 4 hours; and is receiving an IV corticosteroid and antibiotic. He has bilaterally diminished breath sounds with coarse expiratory crackles in both bases. He has a weak nonproductive cough. A chest x-ray for this morning shows bibasilar infiltrates. The current ABG results reveal: pH 7.32, PaCO₂ 57 mmHg, PaO₂ 54 mmHg, SaO₂ 85% and HCO₃ 28 mEq/L.
