J.C is a 40-year-old male who presents today to clinic for complaints of lightheadedness x3 days, starting Saturday (1/10/24) progressively worsening on Sunday morning. On Sunday, had an episode of lightheadedness accompanied by nausea and vomiting. The episodes feel like a “spinning” sensation, triggered by daily activities lasting 2-3 hours. Recalls at the time symptoms aggravated by standing up and bending over to place dishes in dishwasher. Symptoms alleviated by sitting down and stopping movement. Monday episodes have been intermittent, improving now. Does complain of ” brain fog.” Denies recent injury. Secondary complaint of right ear pain described as sharp, 3 out of 10 on pain scale. Ear pain accompanied with congestion, stated he is getting over a cold. Additional complaints of headaches, pain located on frontal area of head, does not radiate. Pain 3 out of 10 on pain scale. Headache does not correlate with episodes of lightheadedness. No history or recurrent headaches. Patient took ibuprofen for both ear pain and headache with relief. Denies personal history or family history of vertigo/ lightheadedness. States he does have a history of anemia being followed by PCP and monitoring. Not taking any medications for anemia.
