A 78-year-old white female with history of frequent hospital admissions for abdominal pain and distention and low-grade fever was admitted to the hospital because of melena and weakness of 24 hours’ duration. She gave a history of having duodenal ulcer disease for approximately 25 years and has been taking anti-ulcer medicines more or less since that time. Her physicians were concerned that she had a perforated ulcer even though her chest and abdominal radiographs failed to show the presence of free air. She had recently increased her use of NSAIDs because of recurrent hip pain, but had lapsed in taking her antacid medications.
