On initial exam performed 2 hours after birth, Baby L is large but appears well. She does have a soft murmur heard at the lower edge of her sternum (breast bone). Her pulses are all normal. Her hands and feet are blue but her lips and tongue are pink. She breast feeds well and falls asleep in her mother’s arms. When the baby is 8 hours old, mother calls her nurse because she has not been able to get baby to breast feed since that first feeding. The nurse notices that the baby no longer appears to be pink and is breathing rapidly. She calls you, the provider, to assess the baby.
