Pt is 58 years old male NKD, he is full code and he was admitted on sick sinus syndrome and transferred to BSNH for pacemaker. Pt diagnose with cardiac arrest/ syncope as well as right plueral effusion, anascara, abdominal ascite, NStemi type 2. Past medical history of CAD, diabtes, hypertension and hypothyroidism. Patient can follow command when is awake. He is on sedation due to mechanical ventilation. Pt receives continuous feeding tubes patient have NG tube on low suction. Skin breakdown and tear on left arm. And lower extremity. Crackle sound on upper lobe on auscultation, bowel sound is normal. Patient have multiple bowel movement last night. Heart sounds are clear on auscultation. Labs ate BUN is 30, Creatinie is 2.9 sodium is 134, calcium is 7.5, total protein is 5.8, phosphate is2.4. RBC 3.3 hgb 9.0 hct 26.4. Patien is Right BKA. Patient medication Cefipine for pneumonia Levothyroxine Propofol Pepcid CT scan result patient might have peritoneal Carcinoma
