Upon arrival to the ED, the patient had no pulse or blood pressure. His rhythm showed a slow neuro complex QRS. Without a pulse, he was determined to be in pulseless CIA. He was bagged and given Atropine and then epinephrine and his pulse was palpable. He immediately went back into V fib and he was shocked. Went back into V fib and was shocked again. Approximately 45 minutes after the initial collapse, there was no response to treatment and efforts were called. He had dilated fixed pupils. Expiration was called at 10:50. ASSESSMENT: Expired. Cause of death cardiac arrest, likely myocardial infarction. Contributing factors of longstanding coronary artery disease, COPD and diabetes. Assign code(s) for facility services. Assign CPT code(s) for facility services.
