Clinical Vignette 2: Annie is a 60-year-old African American female, seeking care from a new provider with no complaints today. She is adherent to her medications. The rest of her HPI is unremarkable. PMHX: Type II DM x 5 years and HTN x 10 years. No history of stroke or CV disease. Past Laboratory Studies (4 months ago) – Serum chemistries, cholesterol, and CBC were normal at that time. BUN = 18 mg/dL. A1C=6.3%. UA: normal pH and specific gravity, negative glucose, negative bilirubin, negative ketones, 1 protein, negative leukocyte esterase, and negative nitrite; ECG was normal Meds: Metformin 1 gm twice a day: Losartan 50 mg PO daily PE: BP 140/88, Wt 170 pounds, Ht 5’2″. Reminder of examination is unremarkable. The NP orders routine labs and reviews them with her one week later. CBC
