R.W is showing symptoms related to her COPD exacerbation, pain related to her osteoarthritis and malnutrition issues related to her poor dentition. The priority nursing diagnosis would be ineffective breathing pattern related to COPD exacerbation AEB decrease in oxygen saturation, heavy cough, thick and yellow sputum and elevated carbon dioxide levels. The patient is having to prop herself up on pillows at night to sleep and has been having poor sleep quality. R.W could possibly benefit from pulmonary rehabilitation which helps people with lung disease learn respiratory exercises and breathing techniques. Monitoring oxygen levels and keeping airways clear of mucous would be other nursing interventions for this patient. If the patient is able to increase fluids to reduce the amount of secretions it would be a good option.
