What will two S.O.A.P. Notes be? A patient presents to ED with acute manic episode acute psychosis. The patients sister states that the patient is intermittenly more angry, more polical, and spending more money over the last 3days. Current medications are Depakote, Seroquel, Zydis, Asenapine and Ativan. Patient has been delusional on the unit however he took his meds and attended groups with no issues. He is preoccupied with communist countries and has a flight of ideas that are nonsensical when talking about it.When the patient woke up the next day he was anxious, disorganized,but calm with staff. That afternoon the patient became delusional with disorganized thought process and nonsensical with flight of ideas. The patient has no bx issues. The same evening the patient became hyperverbal with another flight of ideas but was calm.
