The knife is right next to the spinal cord and the patient is complaining of ipsilateral paresthesias below the point of injury, as well as severe difficulty in breathing which is not being resolved by any of your treatments. You are being assisted by firefighters who estimate that it will take at least 15 minutes to implement SMR that will work with the knife sticking out the patient’s back. Which of the following treatments should you attempt? Perform full SMR using the firefighters’ recommendations as the patient clearly has a primary and maybe a secondary SCI. Stabilize the knife as best you can and then rapidly transport the patient. Perform SMR with the patient in the prone position. Remove the knife so you can just do normal SMR which will save a lot of time.
