Mrs. Phillips is a 45-year-old African-American female. She presents today with complaints of increased thirst, fatigue, increased urinary frequency and nocturia for the past 2-3 months. She denies dysuria, incontinence, and feeling of incomplete bladder emptying or hematuria. She denies fever, chills, chest pain, SOB, abdominal pain, constipation or intolerance to cold. She states she does have numbness/tingling in hands and feet which is also a new symptom but occurs rarely. Denies any recent dietary changes but states she “does like to eat sweets”. Her past medical history is unremarkable. She does not take any medications on a daily bases. NKDA She is a second grade teacher and is married with two children. She denies smoking or illicit drugs. She states she drinks alcohol rarely. Her family hx is positive for Diabetes and HTN in multiple family members.
