Case Study: Emergency Department 1030 Nurse’s Notes: Adam Rickhert, a 59-year-old male with a past medical history of hypertension, hypercholesterolemia, and obesity. Presented to the Emergency Department with chest pain and shortness of breath. Chest pain started while shoveling snow in their driveway. They rate their pain at 7/10, located midsternal and radiating down their left arm and through to their back. Heart sounds S1S2 present and lungs are clear to auscultation. The client is receiving oxygen via nasal cannula @ 2L/min. Vital Signs: BP 140/90 mmHg, HR 93 bpm and regular, RR 32, T 98.4o F, SPO2 90% 2L/min. Labs: Cardiac enzymes pending Chest Xray: Results pending EKG: Normal Sinus Rhythm with ST-Segment elevation. Medications: ASA 324mg PO one-time dose, Nitroglycerin 5 mcg/minute continuous IV infusion. Titrate by 5 mcg/minute IV every 3 to 5 minutes until clinical response, or to a dose of 20 mcg/minute IV.
