A patient being evaluated for a possible diagnosis of cirrhosis has a history consistent with the diagnosis. Which diagnostic test(s) will the health care provider consider initially for additional evaluation? Once a thorough history consistent with cirrhosis is obtained a full liver panel (AST, ALT, alkaline phosphatase, total bilirubin, and serum albumin) and prothrombin time should be obtained. Because no radiographic imaging study is considered the standard, often a right upper quadrant ultrasound is used, due to its absence of radiation exposure and relative low cost to identify gross anatomical changes to liver. The most invasive diagnostic approach is with a liver biopsy, although it is considered the “gold-standard” diagnostic test.
