Physical exam: Sitting in chair in the ICU. Helmet in place. O2 via nasal cannula. Pt asleep upon arrival to room but awoke with verbal greeting. Kept eyes open but did not attempt to vocalize. Did not track clinician’s movements around the room. Did not follow any commands or answer any questions. Speech, language, and voice could not be assessed as pt did not vocalize. Family member present reports that prior to accident, patient was functioning normally without any s/sx of aspiration or other dysphagia. Oral motor exam: Teeth: Intact in fair condition. Secretions: WNL. Right-sided facial weakness appreciated. Could not perform formal oral motor exam given pt not following commands. Opened mouth slightly to allow for passage of POs but was observed with limited lingual and labial movement. Gag absent to Yankauer suction. Based on the information we’ve collected thus far, what do you think her oral phase will be like? What about her pharyngeal phase?
