HPI: 52 year old white male is concerned that he has gained 30 lbs. in 1 year. He has no energy after a day’s work as a real estate agent at a very fast paced company. Last exam was1 year ago; he recalls being told his blood pressure was on the high side once. He has no idea what his BP runs now. Admits to eating “on the run” and “whatever is easy.” Subjective: Meds: •none except for occasional Tums for heartburn. Allergies: •NKDA, food allergies, or environmental… FH: •both parents with HTN •paternal uncle died of MI at 50 years of age •62 year old brother has HTN and had MI 2 yrs ago. Objective: VS: •BP 170/108; HR 88; BMI >40. Diagnostics: QUESTIONS What are his Risk Factors? What are your differentials? or “working diagnoses”? What is your approach to treatment?
