The patient is a 35-year-old nulligravida admitted for excision of condyloma of the vulva. She had a history of papillomatosis, which were excised in 1988. In May, raised papillomatous lesions were noted extending from the clitoris on the left inferiorly, approximately 2 cm and 0.5 cm wide. Biopsies were obtained which confirmed condyloma and marked dysplasia extending to the lateral margins. The patient was admitted for further excision. Pap smear was negative. With the patient in the dorsal lithotomy position, the vagina was prepped and draped in the usual fashion. Acetic acid was applied and the abnormal area of condylomatous changes was easily demarcated. This area was infiltrated, after further prep with Betadine, with 10 cc of 1% Xylocaine. The entire lesion was excised in elliptical fashion with a 5 mm margin on either side. Hemostasis was obtained with the Bovie, and skin edges were approximated with a continuous locked suture of 4-0 Vicryl. Hemostasis was adequate and the patient went to Recovery.
