Coding Assignment HPI: The patient is a 35-year-old white transgender female. She came in today because she has been feeling “anxious and irritable” for several months but has noticed it has been worsening over the past few weeks. She reports being on edge, worrying too much, poor concentration, and having sleep trouble. She reports a personal history of generalized anxiety disorder since age 17. She has been on Lexapro 10 mg and reports it worked well for her in the past without adverse effects. Mental Status Exam: Behavior cooperative, appearance well-groomed, eye contact normal, speech fast, movement within normal limits, mood anxious, thought process circumstantial, no auditory or visual hallucinations, no delusions, no suicidal or homicidal ideation, insight and judgement are intact.Plan: 1. Increase Lexapro to 20 mg PO every morning #30 with 0 refills 2. Patient to follow up with me in 4 weeks 3. Total time spent with patient 30 minutes 4.
