Use the Program Planning formula to evaluate the appropriateness of your proposed change project: My proposed change project is to create biopsychosocial assessment specific to neuromuscular patients for my placement agency. Who= target population (neuromuscular patients), Whom= staff persons (clinicians) 

Match the needs AND resources available

What= Supplies/tools and human capital (Me-the social worker, clinicians, patients, computer, textbooks, paper, printer)

When= scheduling AND lifespan (ALS clinics Jan-March, utilized for the clinics entirety)

Where= Location- appropriate, sustainable, and community focused (at the clinic where the patients come for services)
