Depression Case Study Patient Name: Molly Smith Location: St. Vincent Stress Center, Adult 20 unit (inpatient) Admit Date: 01/20/2022 at 0300 Current Report: Patient in room sleeping, has refused to talk with psychiatrist, and nursing staff. Admission assessment needs completed. History of Present Illness: “I’m just over life, I want to die. It’s all just too hard.” 42 year old, caucasian female admitted to inpatient after overdose of Effexor. Per ED report patient took 30 Effexor 75mg pills at 0200 then she contacted her ex-boyfriend, left a message that said, “Goodbye”, and ex-boyfriend contacted police for a wellness exam. Upon the arrival of police, patient did not answer the door, police entered, and found female patient lying on bathroom floor, difficult to arouse. Patient was taken to St. Vincent ED, had gastric lavage, and has currently been cleared by internal medicine. Transfer was completed at 0600 -1/20/2022. Past psychiatric illness: Seeing outpatient therapist for 10 years. Two prior inpatient admission for suicide attempts, last 2 years ago after a break-up. Medical History: Asthma, Hypercholestermia, Migraines, Depression Current Meds: Simvastatin 40mg PO Q Day, Albuterol Inhaler 2 puffs Q 4 hours PRN SOB, Effexor 150mg PO Q Day. Surgical History: Tonsillectomy in childhood, Left ACL reconstruction 2013, Cholecystectomy 2012 Family History: Only child, Mother has “lots of problems and is depressed”-no diagnosis, Father has anxiety treated with Alprazolam (Xanax), no suicide attempts in family. Never been married, no children. Social History: Lives alone, recent break up with boyfriend. Smokes 1 ppd x 15 years, 4 beers per week “on weekends”, denies illicit substances, has used marijuana in the past-no use for 3 years. No hobbies, no close friends. Has a lab that she walks in the neighborhood daily. Vocational History: LPN at extended care facility for 15 years, enjoys her work. CNA from age 18-27. Educational History: LPN from Ivy Tech at age 27 Spiritual History: Christian, attends church sporadically, no source of distress. Diagnosis: 296.33 Major Depressive Disorder without psychosis 4.
