Nursing Scenarios
1.  Bed 1. Mrs. Smith (name unknown, found on the street with no ID) is a NFA client that was admitted overnight after being found slumped over on a city sidewalk. She was bleeding noticeably from multiple wounds to her body and it was reported that her skin tone appeared “green” and the person believed her to be septic. She just arrived to the unit and needs to have her wounds dressed as ER was too busy to tend to them. They did start her on IV antibiotics (a line had to be started in her foot due to poor veins, the IV team has been contacted to change it tomorrow to a more appropriate site) as she does have a Staph infection in her blood, likely from her wounds. She has approximately 5 wounds, 3 on her legs, another on her left thigh and the last one, very deep, to her left forearm that she keeps picking at.
2.  Bed 2. Mr. Lee, 78 years old had a left THR 7 days ago and has been in the hospital since due to a variety of issues including difficulty voiding and mobilizing. His incision has been healing well and has been kept OTA for the last 2 days. The order is for the staples to be removed on POD7-10. It is expected that he will be discharged home early tomorrow morning. On nightshift he was noted to not sleep well, he was restless and a little confused about where he was.
3.  Bed 3. Mr. Tang is a 72 year old who had a femoral-tibial bypass to both legs (due to peripheral arterial disease) 9 days ago. His discharge home was significantly delayed due to +4 edema after surgery which caused a huge barrier to his mobility. It took many days of trialing modalities to get the swelling down and then for him to learn to mobilize with these very long incisions. His legs still have +1 edema and he is unable to hold them up for very long if he isn’t in a supported position. His staples are due to come out, the physician wrote an order to “D/C alternating sutures today and the rest tomorrow, place SteriStrips PRN.”
4.  Bed 4. Ms. Fernandez, 35 year old had a ganglion cyst removal surgery done to her right wrist 10 days ago. She had gone home after the day surgery but was readmitted last night after she tripped, fell on her newly-operated wrist and hit her head while walking down the stairs in her home. She was admitted for observation and because her wrist was quite swollen and showing signs of fresh bruising. She has NVS ordered q4h. She has been complaining of a headache and concerns of pain to her R wrist. There is a Physician’s order to have the sutures to her wrist be removed during her stay. She is likely to be discharged home tomorrow as her assessments have all been WNL and her wrist swelling has decreased significantly with ice and elevation. She has Acetaminophen and Ibuprofen PRN ordered for pain control.Â
