Case Scenario B – Rabies
Ms. Annie Malbite, a 28-year-old, veterinarian visits an out-patient department facility with a laceration on the right ankle. The laceration occurred 1 month prior to the consult and was due to a bite of a cat that was being observed in the facility that the patient was working. The laceration was violaceous in color, measuring 6 cm with pus around the periphery of the wound. Together with the laceration were two puncture sites at the lateral aspect of the ankle. Upon further assessment and interview, Ms. Annie Malbite mentioned that she cleaned her wound with running water and applied garlic to the wound. 1 week after the incident, she went to a nearby health center and was given one dose of Tetanus Toxoid Injection. She is also drinking Paracetamol every 4 hours and no other medications are taken. She now goes to you because of a change in behavior. She verbalizes that “nauulol na ata ako.” She is accompanied by her workmate and says that the patient is exhibiting a change in behavior a week prior to this consult. Her workmate shares: “She is acting strange; she is not like that before. She is afraid of water and screams even with the slightest waft of air felt.” Her vital signs are as follows: Temp: 38.8 degrees Celsius, Respiratory Rate: 19 BPM, Pulse Rate: 98 BPM, and a BP of 90 mmHg.
1. Hyperthermia related to infectious process as evidenced by elevated body temperature of 38.8 degrees Celsius. – Core body temperature above the normal diurnal range due to failure of thermoregulation.
2. Ineffective breathing pattern r/t anxiety – Inspiration and/or expiration that does not provide adequate ventilation.
3. Risk for injury r/t altered mental status – susceptible to physical damage due to environmental conditions interacting with the individuals’ adaptive and defensive resources, which may compromise health.
4. Impaired physical mobility r/t altered mental status – Limitation in independent, purposeful physical movement of the body or of one or more extremities.
5. Deficient knowledge r/t insufficient information – absence of cognitive information related to specific topic.
6. Readiness for Enhanced Family Coping – A pattern management of adaptive tasks by primary person (family member, significant other, or close friend) involved with the client’s health challenge, which can be strengthened.Â
*Identify cue clusters or clinical manifestations that are associated with the identified given NURSING diagnosis.
