Mrs. C. D. is a 91-year-old who was admitted to a long-term care facility after her stroke 6 months ago. She is unable to bathe, dress, feed herself and is incontinent of bowel and bladder. She also has difficulty speaking and swallowing, and is on Aspiration Precautions. Â Prior to her admission to the long-term care facility, she lived by herself and was very active in the community and church.
For the past few days, she has become very angry and even combative during care. She also refuses to eat or drink anything that is offered. Mrs. C. D.’s daughter, power of attorney (POA), is notified and a special care meeting is scheduled.
The day of the meeting Mrs. C. D., her daughter and son are in attendance. The physician suggests a feeding tube to maintain adequate hydration and nutrition. Her son says “Absolutely! Not feeding her is almost like we are killing her. I do not think we should play God.” When Mrs. C. D. is asked if you wanted a feeding tube, she just stares out the window. The daughter, POA, explains that Mrs. C. D. had a living will drawn up which “states No artificial measures, including a feeding tube, should be used to keep her alive.” Both of Mrs. C. D.’s children agree to follow their mother’s wishes since they were made while she was still in a good state of mind.
